This presentation explores how ACSO supports continuity of reintegration for more than 2,200 men exiting prison each year across Queensland, with planning commencing three months prior to release. It highlights the importance of collaborative partnerships between corrections, community services, and local organisations in managing risk while maintaining continuity of care across geographically dispersed and culturally diverse communities. Drawing on regional case studies, the session will demonstrate how coordinated, community-based responses support safe return to Country, strengthen access to housing, employment, social and cultural supports, and enable reintegration planning to continue beyond custody. The presentation will also examine how pre-release preparation, LS:RNR-informed assessment, and strong escalation pathways contribute to a wraparound approach that improves safety, continuity, and successful reintegration outcomes.
