Health through Walls uses Digital Adherence Technology (DAT) as an alternative solution to Directly Observed Therapy (DOT) in five (5) Haitian prisons with little to no healthcare staff.
Digital adherence technology is innovative:
- In a prison setting where such technologies are generally absent or even prohibited due to security concern.
- In improving treatment adherence and outcomes for TB by engaging the correctional officer to participate in TB treatment. It allows a greater reach of treatment for prisoners with TB in areas where healthcare staff are limited.
The Video Observed Therapy (VIOT) project was successful in showing good adherence and treatment outcomes among prisoners under VOT as compared to DOT. The impact of the COVID-19 pandemic has enabled easier access of new technologies into the prison system.

Dr. Edwin Prophete MD (Health through Walls) is a staff physician at Health through Walls, Port-au-Prince. He currently serves as the Deputy Medical Supervisor and was the project director for TB Reach Haiti, a program that implemented Video Observed Therapy in the prisons of Haiti.
