The Singapore Prison Service (SPS) adopts a throughcare approach to rehabilitation and reintegration where there are continuous and coordinated efforts to address the risks and needs of inmates while they are in prison and upon their release into the community. SPS's Community Mobilisation approach demonstrates how multi-stakeholder partnerships in the community create a strong rehabilitation and reintegration ecosystem. This presentation showcases the network of support available for inmates and ex-offenders on their throughcare journey, highlighting initiatives such as the Desistor's Network (DN), the Family Engagement Model, the CARE Network, and how other community partners come together to provide step-down care and essential reintegration support. Through family-centric interventions, peer support systems, and coordinated community partnerships, SPS works towards improving offenders' wellbeing and rehabilitation outcomes.

Luke Leong has been with the Singapore Prison Service since 2010. He has completed stints in operations and policy work, including postings with the Ministry of Home Affairs and Ministry of Foreign Affairs. He is currently the Superintendent of Institution Tanah Merah, which houses Singapore’s Prison School and Reformative Training Centre.
